Transcript Request Form

Please note: this request is only for students to obtain a copy of their official transcript. School districts must request records for students currently attending Quaker Digital or Quaker Prep by sending a records request to: records-request@quakeracademies.org.

Copies of your official academic transcript are available through the following delivery methods:

  • Mail: transcripts can be mailed at no cost.
  • Email: transcripts can be email at no cost.

Some academic institutions only accept transcripts sent directly to them. Please check with the academic institution for where and how the transcripts can be delivered.

Please allow two business days for your request to be processed. Requests are processed in the order they are received. Delays may occur during peak seasons (the start and end of the current school year).
